Title: La experiencia y vivencia del aprendizaje clínico en el Grado en Enfermería. Estrategia para la mejora de la calidad docente

Abstract: [spa] El objetivo principal fue comprender las vivencias y experiencias de los estudiantes de Enfermería, tutores de prácticas y enfermeras asistenciales durante el aprendizaje clínico. Se planteó un estudio mixto utilizando el principio de la triangulación a través de diferentes instrumentos (entrevistas, grupos focales y cuestionario), diferentes agentes (enfermeras asistenciales docentes, tutores y estudiantes de prácticas) y diferentes métodos (métodos cualitativo y cuantitativo). El ámbito de estudio fue el Campus Docent Sant Joan de Déu-Fundació Privada. La metodología cualitativa fue de diseño fenomenológico con un muestreo no probabilístico, estratégico o intencional. Los participantes fueron 51, 28 eran estudiantes de prácticas de 2º, 3r y 4º curso; 8 Enfermeras asistenciales y 15 tutora/es de prácticas. Se identificaron los códigos y se agruparon en categorías y subcategorías. Para el análisis de los datos se utilizó el programa Atlas.ti 7.1.7 ®. En el diseño cuantitativo, se realizó un estudio descriptivo transversal para identificar los factores que producen preocupación y estrés en las estudiantes antes de la práctica clínica. La población de estudio fueron los estudiantes (n=428) del Grado en Enfermería del Campus Docent Sant Joan de Déu matriculados en las asignaturas prácticas, finalmente participaron 390. Para la recogida de los datos se diseñó un cuestionario ad hoc, formado por la información de variables relacionadas con las características socio- demográficas, laborales y formativas, y otro apartado recogía información a través del cuestionario Kezkak. Para el análisis de los datos, se utilizó el paquete estadístico SPSS (SPSS versión 22; Inc., Chicago, Illinois, EE. UU). Los estudiantes presentaron preocupaciones y factores de estrés antes y durante las prácticas clínicas. Las expectativas de las estudiantes ante cualquier inicio de prácticas están determinadas por la ilusión y motivación, que contrastan con la incertidumbre y la preocupación ante lo desconocido. Las interacciones entre estudiantes, tutoras y enfermeras están determinadas por la actitud, compromiso, interés y capacidad de adaptación de las estudiantes. El significado de las prácticas clínicas es transcendental para estudiantes, tutoras y enfermeras. La reflexión y el debate de éstas ha permitido una revisión de la práctica docente que se ha concretado en propuestas de mejora para el aprendizaje clínico.
[eng] This thesis had two main objectives. The first was to know the experiences of nursing students, training tutors and referent health care nurses during clinical training. The second general objective was to identify the process of the acquisition of professional competences. A mixed study was designed using the principle of triangulation through different instruments (interviews, focal groups and questionnaire), different agents (nursing teachers, tutors and students in training) and different methods (qualitative and quantitative methods). The study setting was the Campus Docent Sant Joan de Déu-Fundació Privada, The design of the qualitative methodology was phenomenological. The system of non probabilistic, strategic or intentional, sampling was used. 51 participants were included, 28 being students in training of the 2nd, 3rd and 4th year of Nursing Degree; 8 were referent health care nurses from different settings and 15 tutors. For the analysis of the qualitative data, Atlas.ti, version 7.1.7® was used. This program complemented and helped to classify and order the different categories and subcategories. A transversal descriptive study was carried out for the quantitative analysis to identify the factors producing worry and stress in students prior to clinical training. The study population included all the students (n=428) studying the Degree of Nursing who were registered for the clinical training classes. Thus, a total of 390 students were finally included in the study. An ad hoc questionnaire was designed for data collection, with 2 sections; information of variables related to socio-demographic, occupational and educational characteristics and the second collected using the Kezkak questionnaire. The statistical analyses were performed using the statistical package SPSS (SPSS version 22; Inc., Chicago, Illinois, USA). Students have worries and stress factors before and during the clinical training practices. The expectations of the students before clinical training is determined by their enthusiasm and motivation which are in contrast to uncertainty and anxiety towards the unknown. Interactions among students, nurses and tutors are determined by the attitude, commitment, interest and the capacity of adaptation of the students, The significance of clinical training is transcendental for students, tutors and nurses.
