dc.description.abstract[spa] INTRODUCCIÓN: El "Staphylococcus aureus" resistente a meticilina (SARM) es un patógeno de gran importancia para la Salud Pública tanto a nivel hospitalario como en la comunidad. Presenta una rápida diseminación por todo el mundo y crea resistencias a los diferentes antibióticos con gran facilidad. En España se describió por primera vez a finales de 1970 y su frecuencia hasta nuestros días ha seguido un curso ascendente en el que se han producido cambios epidemiológicos y moleculares destacables. Con objeto de conocer la situación actual del SARM en España, se realizó un estudio multicéntrico durante el período de junio de 2003 en el que participaron 64 hospitales españoles pertenecientes a 14 Comunidades Autónomas. OBJETIVOS: Se determinó el porcentaje de resistencia a meticilina en las cepas aisladas durante dicho período, se estudió la sensibilidad a otros antibióticos no beta-lactámicos, así como las propiedades genotípicas de las cepas SARM, la detección de genes de resistencia a macrólidos y finalmente el estudio de la producción de leucocidina de Panton-Valentine en aquellas cepas de adquisición comunitaria y en una selección de cepas de origen nosocomial. METODOLOGÍA: caracterización bioquímica de microorganismos, técnicas microbiológicas convencionales, métodos moleculares de ácidos nucleicos como PCR, Electroforesis en campo pulsátil (ECP), Multilocus sequence typing (MLST), Staphylococcal cassette chromosome mec (SCCmec). Detección de factores de virulencia. RESULTADOS y CONCLUSIONES:- El porcentaje de resistencia a meticilina fue del 20% (similar al obtenido en otros estudios realizados en nuestro país y en Europa).- No se detectaron cepas resistentes a glucopéptidos, aunque un 1,4% de las cepas presentaron heterorresistencia a vancomicina. - El 60% de las cepas resistentes a eritromicina presentaron un fenotipo MLSB codificado por los genes ermA y ermC y el 40% restante presentaron un fenotipo MSB codificado por el gen msrA. - Se identificó un complejo clonal dominante, 73% de las cepas, (ST125::P/Q::mecIV) cuyo ancestro genético podría estar relacionado con el clon epidémico internacional denominado Clon Pediátrico (ST5::mecIV). Estaba presente en todos los hospitales participantes en el estudio.- En menor frecuencia se detectaron otros clones epidémicos, EMRSA-16 (ST36::mecII), 8%, aislado preferentemente en la comunidad Gallega; EMRSA-15 (ST22::mecIV), 2%, aislado en un hospital de Palma de Mallorca; y un solo aislamiento del Clon Ibérico (ST247::mecI) aislado en un hospital de Toledo. - La prevalencia de los aislamientos de SARM de origen comunitario, productores de leucocidina de Panton-Valentine fue baja (3%). Las tres cepas fueron aisladas en dos hospitales en Barcelona y pertenecieron al mismo clon (ST8::BG::mecIV).spa
dc.description.abstract[cat] "EPIDEMIOLOGY OF RESISTANCE TO METHICILLIN IN STRAINS OF Staphylococcus aureus ISOLATED AT SPANISH HOSPITALS"INTRODUCTION: Methicillin-Resistant Staphylococcus aureus (MRSA) is one of the most relevant health care-associated pathogens. The spread of new antibiotic resistance patterns among methicillin-resistant Staphylococcus aureus (MRSA) and the detection of the first cases of infection by community-acquired (CA) MRSA, are among the most important features exhibited by MRSA in recent years in Spanish Hospitals.OBJECTIVES: A multicenter study was carried out in Spain during the period of June 2003 to evaluate the percentage of resistance to methicillin, the current levels of antibiotic resistance, the detection of genes of resistance to macrolides, the distribution of clones and the prevalence of strains producing Panton-Valentine Leucocidine and CA-MRSA. METHODS: Biochemical characterization, Molecular typing of MRSA isolates by Pulsed-field gel electrophoresis (PFGE), multilocus sequence typing (MLST) and Staphylococcal cassette chromosome mec (SCCmec). Production of Panton-Valentine Leucocidine by PCR.RESULTS AND CONCLUSION:- The percentage of methicillin resistance was to 20%. - There were no glycopeptide resistance, although 1.4% of the isolates showed vancomycin heteroresistance. - Resistance to macrolides of MLSB phenotype was found in 60% of MRSA strains. This was due to the presence of ermA and ermC. In 40% was detected MSB phenotype, these strains carried the genetic determinant msrA, responsible of an active erythromycin efflux. - It was detected a clonal complexe dominant, 73% of the isolates, (ST125::P/Q::mecIV) whose ancestral genetic background would be able to be pertaining to the epidemic international clone named Pediatric clone (ST5::mecIV). He was present at all of the participating hospitals in the study.- More rarely were detected another epidemic clones, EMRSA-16 (ST36::mecII), 8%, isolated preferentially in Galicia; EMRSA-15 (ST22::mecIV), 2%, isolated at Palma's hospital of Mallorca; And only one isolated of the Iberic clone (ST247::mecI) was found among the MRSA studied, isolated at Toledo's hospital.- The prevalence of CA-MRSA was very low in this period (3%). Three strains were isolated at two hospitals in Barcelona and belonged to the same clone (ST8::BG::mecIV ).cat
