dc.description.abstract[spa] OBJETIVO: La simpaticolisis torácica endoscópica bilateral (STEB) es un tratamiento eficaz y mínimamente invasivo como tratamiento de la hiperhidrosis primaria. Nuestro objetivo es evaluar el nivel de ansiedad subjetivo y objetivo mediante una escala psicométrico estandarizada antes y después de la intervención.PACIENTES Y MÉTODO: Un total de 106 pacientes diagnosticados de hiperhidrosis primaria y tratados mediante STEB fueron estudiados mediante un cuestionario preoperatorio y otro postoperatorio a los 12 meses de la intervención sobre: síntomas asociados a la hiperhidrosis, nivel subjetivo de ansiedad así como efecto de la misma sobre la vida diaria. Paralelamente se les administró el cuestionario STAI (Cuestionario de ansiedad general Rasgo y Estado) y ICVD (Índice de Calidad de Vida en Dermatología).RESULTADOS: Respecto a la sintomatología general asociada se observó como las palpitaciones se registraron preoperatoriamente en el 39,62% de los casos versus el 10,37% postoperatorio; temblor de manos en el 23,58% versus 8,49%; enrojecimiento facial en el 54,71% versus 11,32%, cefalea en el 29,24% versus 9,43% y epigastralgia inespecífica en el 18,86% versus 6,6%; respectivamente. El nivel de ansiedad subjetiva referida por los pacientes sobre una escala comprendida entre 0 (no ansiedad) y 4 (ansiedad incapacitante) fue de 2,08 +/- 1,1 preoperatoriamente versus 0,39+/- 0,67 postoperatoriamente (p<0,001), y el impacto sobre la vida diaria de la hiperhidrosis antes y después de la intervención también experimentó un descenso estadísticamente significativo (p<0,001). Los resultados obtenidos mediante el cuestionario de ansiedad general STAI indican como preoperatoriamente existe tanto una ansiedad-estado como ansiedad-rasgo elevada, estadísticamente significativa, respecto postoperatoriamente (p<0,001). Mediante el cuestionario ICVD se demuestra como preoperatoriamente el disconfort en la calidad de vida es "elevado" siendo la mejora estadísticamente significativa (p<0,001).CONCLUSIONES: Los pacientes con hiperhidrosis primaria tratados mediante STEB presentan una sintomatología general y un nivel de ansiedad preoperatoria elevado respecto a la postoperatoria, tanto en la valoración subjetiva del paciente como mediante instrumentos psicométricos estandarizados, aunque no sean específicos para valorar la hiperhidrosis primaria.spa
dc.description.abstract[eng] OBJECTIVE: Endoscopic bilateral thoracic sympathicolysis (EBTS) is an effective and minimally invasive procedure used for patients with primary hyperhidrosis. The purpose of this study was to examine anxiety levels using standardized psychometric tools in hyperhidrosis patients before and after EBTS . METHOD: A total of 106 patients diagnosed with hyperhidrosis who underwent EBTS were asked to fill out a questionnaire before and twelve months after the procedure that elicited the following information: a) symptoms associated with hyperhidrosis, b) the patient's level of anxiety, and c) the extent to which this anxiety was incapacitating in their daily life. All patients also completed State-Trait Anxiety Inventory (STAI) and Dermatology Life Quality Index (DLQI) before and twelve months after the EBTS. RESULTS: Palpitations were reported preoperatively by 39.62% of patients versus 10,37% postoperatively, trembling of the hands in 23,58% versus 8,49%, facial blushing in 54,71% versus 11,32%, headache in 29,24% versus 9,43% and non-specific epigastric pain in 18,86% versus 6,6%, respectively. Patients reported a marked improvement in the level of anxiety from a mean SD of 2.08 +/- 1.1 preoperatively versus 0.39 +/- 0.67 postoperatively (p<0.001), and the social impact (debilitating) of primary hyperhidrosis before and after surgery also showed significant improvement (p<0.001). The results of STAI showed significant improvement in the levels of anxiety after surgery compared with the preoperative levels and with established norms (p<0.001). Treatment with EBTS led to improvement in quality of life in all patients, with the median DLQI sore decreasing significantly from pretreatment level (p<0.001). CONCLUSIONS: Patients with primary hyperhidrosis that undergo EBTS presented a decrease in the level of anxiety and associated symptoms.eng
