dc.description.abstract[cat] INTRODUCCIÓ: El dolor lumbar és una síndrome d'etiologia multifactorial que afecta al 80% de la població com a mínim una vegada al llarg de la vida. Degut a la manca de coneixement de l'element causal del dolor, aquest també s'anomena lumbàlgia crònica inespecífica (CLBP). Existeixen en la literatura múltiples estudis respecte les disfuncions musculars de la població amb CLBP, però amb resultats molt diferents i fins i tot oposats. Per altra banda, no existeix una opció clara i unificada en la valoració i el control d'aquestes disfuncions. OBJECTIUS: Presentar i valorar dues eines pel control i valoració de la musculatura del tronc: una eina per a valorar la força isomètrica màxima voluntària (MVC) i una eina per a valorar les múltiples i subtils activacions i desactivacions musculars que es donen en un procés d'estabilització muscular. Al mateix temps, revisar pràctiques metodològiques utilitzades freqüentment en la valoració de la població amb CLBP: utilització d'un pes proporcional a la MVC com a càrrega relativa a l'individu i utilització d'un pes calculat a partir de les mesures antropomètriques d'aquest. Posteriorment, valorar a partir de l'electromiografia (EMG) de superfície algunes de les disfuncions associades a la població amb CLBP que més controvèrsia mostren en la literatura: fatigability i patrons d'activació muscular alterats. Igualment, valorar la similitud d'algunes d'elles amb les disfuncions que la població sana pugui presentar en situació de fatiga. DISSENY: Estudi de validació i fiabilitat de les dues eines desenvolupades i comparació de les dues metodologies revisades en aquest treball. Igualment, estudi transversal amb grup control (comparació de CLBP vs població sana). Anàlisi mitjançant dos tests: un test de fatiga (contracció isomètrica mantinguda, Sorensen Test) i un test per a valorar les característiques de reacció muscular davant d'una pertorbació sobtada. Enregistrament de la musculatura mitjançant EMG de superfície i control de la postura durant els tests amb un sistema d'enregistrament per càmeres de vídeo. Valoració de paràmetres electromiogràfics de fatiga, amplitud de senyal i latències d'activació de la musculatura de tronc i les extremitats, a la vegada que valoració d'angles, alçades i alienacions corporals. PARTICIPANTS: 26 subjectes amb lumbàlgia crònica 26 subjectes sans en edats compreses entre 25 i 55 anys. RESULTATS: Ambdues eines van mostrar una alta fiabilitat i validesa comparativament al gold standard utilitzat en cada cas. Ambdues metodologies utilitzades van mostrar resultats oposats en la determinació de l'existència de fatígabílity en la població amb CLBP respectivament a la població sana. La població amb CLBP, a partir de la utilització de les eines presentades en aquest treball i el mètode considerat més adequat (càrrega relativa a partir de les mesures antropomètriques), va mostrar presentar nivells de fatígability similars a la població sana igualment que uns patrons musculars diferenciats a aquesta població tant en les seves característiques temporals (major latència d'activació entre altres) com en les seves característiques d'amplitud (major activitat muscular). El patró muscular utilitzat pels CLBP davant la càrrega inesperada, presentava similituds amb el patró muscular utilitzat per la població sana amb fatiga pel que fa a les característiques temporals. DISCUSSIÓ I CONCLUSIONS: Les eines i la metodologia desenvolupades en aquest treball representen una millora en l'estudi de la musculatura de la columna. A més a més, es demostra la importància de les metodologies utilitzades, ja que poden afectar en els resultats d'una forma considerable. El comportament muscular de la població amb CLBP tant en una situació d'isometria mantinguda com en l'estabilització d'una pertorbació es diferencia de la població sana podent ser una disfunció que porti a la cronificació del dolor. La població amb CLBP podria presentar un estat neuromuscular inicial als test similar a la població sana fatigada, tot i que la velocitat de fatigar-se durant un esforç isomètric (fatigabilíty) no va mostrar diferències significatives entre poblacions.-
dc.description.abstract[eng] INTRODUCTION: Back pain is a syndrome with multifactorial etiology that affects 80% of the population at least once in their lifetime. Due to the lack of knowledge concerning its causal factors, it is also known as non-specific chronic low back pain (CLBP). There are multiple studies in research literature regarding muscle dysfunctions of CLBP subjects, but very different results have been observed, which can even be contradictory at times. Furthermore, there is no clear and unified direction regarding the assessment and monitoring of this dysfunction. OBJECTIVES: To present and evaluate two tools for monitoring and assessing trunk muscles: a tool to evaluate maximum voluntary isometric strength (MVC) and a tool to evaluate the multiple subtle muscle activations and deactivations that occur during the process of muscular stabilization. At the same time, the study aims to review some of the methodological practices commonly used in the assessment of CLBP subjects: using a weight proportional to the MVC as the load to be applied to the individual and using a weight calculated from the individual's anthropometric measurements. Subsequently, some of the dysfunctions associated with CLBP subjects which are most controversial in research will be evaluated using surface electromyography (EMG) procedures: fatigability and altered muscle activation patterns. The study will equally address the similarity of some of these with dysfunctions which healthy subjects may experience in situations of fatigue. DESIGN: A validation and reliability study of the two tools developed, together with a comparison of the two methods are reviewed in this paper. At the same time, the study is transversal with the inclusion of a control group (CLBP subjects compared to healthy subjects). The analysis was carried out using two tests: a fatigue test (maintained isometric contractions, Sorensen test) and a test to evaluate the characteristics of muscular reaction to a sudden increased load. Muscles were recorded by surface EMG, and posture control during the tests was filmed by a video camera recording system. EMG fatigue parameters, EMG signal amplitude and latencies of trunk and limb muscle activation were evaluated together with body angles, heights and positions. PARTICIPANTS: 26 subjects with chronic low back pain and 26 healthy subjects, all between the ages of25 and 55. RESULTS: Both tools showed high reliability and validity in comparison to the gold standard used in each case. Both methodologies used showed opposite results in the determination of the existence of fatigability in CLBP subjects compared to healthy subjects. From the use of the tools presented in this work and the method considered most appropriate, (proportionate loads based on anthropometric measurements) CLBP subjects showed increased levels of fatigability similar to those of the healthy subjects, as well as demonstrating muscular patterns that were different from the healthy subjects both in their temporal characteristics (higher latency activation among others) and their amplitude characteristics (greater muscular activity). The muscular pattern used by CLBP subjects when faced with the unexpected load was similar to that of the fatigued healthy subjects fatigue as far as temporary characteristics were concerned. DISCUSSION AND CONCLUSIONS: The tools and methodology developed in this work represent an advance in the study of spinal muscles. Furthermore, the importance of the different methodologies used is demonstrated, as this can affect the results obtained in a significant way. The muscular behaviour ofCLBP subjects, both in a maintained isometric situation as well as in the stabilization process following an increased load, differs from the healthy population, which could well be a dysfunction that leads to the pain becoming chronic. CLBP subjects present an initial neuromuscular state in the test which is similar to that of the fatigued healthy subjects, although the rate of fatigue during isometric effort (fatigability) showed no significant differences between the two groups.-
