To the Editor: Wenzel and colleagues (July 26 issue) recommend long-term azithromycin for a 55-year-old man with recurrent exacerbations of chronic obstructive pulmonary disease (COPD), a forced expiratory volume in 1 second (FEV1) of 50% of the predicted value, dyspnea, cough, and sputum production. However, before azithromycin, we think this patient should be treated with other pharmacologic agents to manage his COPD and prevent exacerbations.-
