Efecte d’un producte probiòtic a base de Faecalibacterium prausnitzii per prevenir les recurrències en pacients amb infeccions del tracte urinari recurrents: Estudi clínic, aleatoritzat, doble cec, controlat amb placebo

Abstract

[cat] Introducció: Les infeccions del tracte urinari (ITU) són una de les malalties bacterianes més freqüents a nivell mundial, afectant principalment a les dones i generant una gran càrrega sanitària, amb una prevalença elevada de recurrències. Recentment s’ha observat que la composició de la microbiota intestinal influeix en l’aparició d’ITU a través de reservoris intestinals d’uropatògens. Mètodes: S’ha dissenyat un estudi clínic d’intervenció aleatoritzat, doble cec, paral·lel i controlat amb placebo per avaluar l’efecte d’un producte probiòtic a base de Faecalibacterium prausnitzii en la prevenció de recurrències en pacients amb ITU recurrents. El producte consisteix en 500 mg de pols amb 107 Unitats Formadores de Colònies (UFC) de F. prausnitzii. S’inclouran 100 dones (50 en el grup probiòtic i 50 en el grup placebo) d’entre 18 i 52 anys que hagin patit un episodi d’ITU amb dues o més recurrències en sis mesos, o tres recurrències en un any (ITU recurrent). Seran tractades durant 12 mesos segons el tractament assignat i seguiran un pla de dieta mediterrània. En total hi hauran cinc visites presencials. La variable principal serà el percentatge de pacients sense recurrències durant el període d’investigació. Com a variables secundàries, s’analitzarà la composició de la microbiota intestinal a partir de mostres fecals, examinant el gen 16S ribosomal (rRNA) per determinar el percentatge de F. prausnitzii i la biodiversitat microbiana específica mitjançant l’índex de Shannon. També s’estudiaran les puntuacions del qüestionari de salut SF-36 per avaluar la qualitat de vida de les pacients. En cas de recurrència, s’avaluarà el temps que el producte retarda l’aparició d’una nova infecció i la simptomatologia mitjançant el qüestionari Acute Cystitis Symptom Score. Finalment, s’avaluaran els possibles símptomes d’ITU, efectes adversos associats al tractament i l’adherència al mateix. Resultats esperats: Els resultats obtinguts permetran determinar l’eficàcia i seguretat del producte probiòtic en pacients amb ITU recurrents. S’espera que en el grup actiu hi hagi una reducció de les recurrències i/o un retard en l’aparició de noves ITU recurrents, així com una millora de la qualitat de vida de les pacients.

[eng] Introduction: Urinary tract infections (UTIs) are one of the most common bacterial diseases worldwide, affecting mainly women and generating a high health burden, with a high prevalence of recurrences. It has recently been observed that the composition of the intestinal microbiota influences the occurrence of UTI through intestinal reservoirs of uropathogens. Methods: A randomized, double-blind, parallel, placebo-controlled clinical intervention study was designed to evaluate the effect of a probiotic product based on Faecalibacterium prausnitzii in the prevention of recurrences in patients with recurrent UTIs. The product consists of 500 mg of powder with 107 Colony Forming Units (CFU) of F. prausnitzii. 100 women (50 in the probiotic group and 50 in the placebo group) between 18 and 52 years of age who have had an episode of UTI with two or more recurrences in six months, or three recurrences in one year (recurring UTI). They will be treated for 12 months according to the assigned treatment and will follow a Mediterranean diet plan. In total there will be five face-to-face visits. The main variable will be the percentage of patients without recurrences during the research period. As secondary variables, the composition of the intestinal microbiota will be analysed from faecal samples, examining the 16S ribosomal gene (rRNA) to determine the percentage of F. prausnitzii and the specific microbial biodiversity using the Shannon index. SF-36 health questionnaire scores will also be studied to assess the patients' quality of life. In case of recurrence, the time that the product delays the appearance of a new infection will be assessed, together with the recurrence symptoms using the Acute Cystitis Symptom Score questionnaire. Finally, possible UTI symptoms, adverse effects associated with the treatment and adherence to it will be assessed. Expected results: The results obtained will make it possible to determine the effectiveness and safety of the probiotic product in patients with recurrent UTI. It is expected that the active group will show a reduction in recurrences and/or a delay in the appearance of new recurrent UTIs, as well as an improvement in the patients' quality of life.
