Abstract: | [spa]INTRODUCCIÓN:
La preeclampsia y la restricción de crecimiento fetal comparten una base fisiopatológica común con una enfermedad placentaria similar y enfermedad materno-fetal variable. La principal hipótesis de esta tesis es que las enfermedades placentarias podrían afectar la estructura y función vascular de madres y neonatos.
MÉTODOS:
El objetivo de este estudio es determinar diversos índices vasculares estructurales y funcionales en madres y recién nacidos de casos de preeclampsia y restricción de crecimiento fetal, como marcadores de enfermedad vascular subclínica y potencial factor de riesgo a largo término.
RESULTADOS:
Tanto los casos de preeclampsia como de restricción de crecimiento fetal presentan cambios vasculares relevantes tanto en madres como en neonatos. Los cambios vasculares maternos se presentan principalmente en aquellos casos de debut clínico precoz. Por otra parte, los cambios vasculares neonatales están presentes independientemente del grado de severidad clínica.
CONCLUSIONES:
Los casos de preeclampsia y restricción de crecimiento intrauterino precoz presentan un remodelado vascular similar, confirmando el concepto de una enfermedad placentaria común que determina cambios vasculares maternos similares. La presencia de cambios vasculares neonatales independientemente del grado de severidad clínica, indica que probablemente la mayoría de fetos pequeños para la edad gestacional presentan programación cardiovascular. [eng]INTRODUCTION: Preeclampsia and fetal growth restriction share a pathophysiological basis due to a common placental etiology and variable maternal-fetal disease. The main hypothesis of this thesis is that placental disease may affect vascular structure and function in mothers and offspring. METHODS: The objective of this study is to determine various structural and functional vascular indices in mothers and neonates affected by preeclampsia and fetal growth restriction, as markers of subclinical vascular disease and potential long term risk factors. RESULTS: Both cases of preeclampsia and fetal growth restriction presented significant vascular changes in both mothers and infants. Maternal vascular changes occur primarily in cases of early-onset disease. Moreover, neonatal vascular changes are present irrespective of the severity of placental affectation. CONCLUSIONS: The cases of preeclampsia and early intrauterine growth restriction present a similar vascular remodeling, confirming the concept of a common placental disease. The presence of neonatal vascular changes indicate that probably the majority of small for gestational age fetuses have cardiovascular programming irrespective of the degree of clinical severity. |
