Molecular Determinants of Early-Onset Thyroid Lesions: the miRNA Biogenesis Pathway as a Main Player

[eng] Thyroid nodules are a common clinical finding, with most being benign. However, certain nodules may progress to malignancy, posing significant clinical challenges for diagnosis, management, and treatment. Familial and early-onset thyroid disorders, such as familial euthyroid thyroid follicular nodular disease (TFND), are of particular concern, as they may signal a hereditary predisposition to thyroid cancer. While several well-characterized hereditary syndromes are known to be associated to thyroid nodule development, many susceptibility genes remain unknown, and the full genetic landscape remains poorly understood. Notably, DICER1, a key enzyme in microRNA (miRNA) biogenesis, has been implicated in both the development of familial TFND and pediatric thyroid tumors. Mutations in DICER1 and other miRNA-processing genes, such as DGCR8, disrupt miRNA processing, dysregulate gene expression, and contribute to tumorigenesis. Yet, the molecular mechanisms underlying the progression from benign lesions to malignancy remain unclear. Therefore, identifying specific susceptibility genes and understanding the mechanisms behind benign to malignant progression are critical for early diagnosis and personalized treatment. This doctoral thesis aims to elucidate the genetic and molecular underpinnings of these diseases through three primary objectives: (i) the identification of novel candidate susceptibility genes involved in the development of familial euthyroid TFND; (ii) the characterization of the molecular profile of pediatric benign and malignant thyroid nodules in a Spanish series in order to identify susceptibility genes involved in the early-onset development of thyroid tumors; and (iii) the investigation of the mechanisms underlying the progression from a benign thyroid nodule to a thyroid cancer progression in DICER1-mutated and DGCR8-mutated thyroid lesions. For the first objective, we profiled the germline of affected individuals from 9 families with TFND by whole exome sequencing. We identified pathogenic alterations in two validated TFND-susceptibility genes (DICER1 and SLC26A4) in 2 families and propose MTHFR, WFS1, SELENOS and RB1 as novel candidate TFND susceptibility genes in 4 other families, highlighting the genetic heterogeneity involved in the etiology of the disease. In the second objective, we performed whole exome sequencing in a series of 33 benign and malignant pediatric thyroid lesions obtained from a single Spanish pediatric hospital. We fully characterized the drivers involved in the development of 13 benign thyroid lesions, identifying drivers associated to known hereditary tumor predisposition syndromes (DICER1 and PTEN), to thyroid hormone synthesis and function (TPO, IYD, DUOX2, and TSHR) and to key signaling pathways in thyroid cancer (RAS and BRAF). Profiling of the 20 malignant thyroid lesions revealed alterations in canonical drivers of the MAPK pathway (BRAF and RET fusions) and in DICER1 while a subset of pediatric thyroid cancers lacked identifiable drivers. These findings reinforced the pivotal role of DICER1 in the development pediatric thyroid lesions. The third objective involved a multiomic characterization of benign and malignant thyroid lesions with mutations in DICER1 and DGCR8. Our findings reveal a progressive, specific, and linear accumulation of genetic alterations, which when combined with enhanced downregulation of miRNAs distinguished DICER1/DGCR8-malignant lesions from their benign counterparts. Compensatory hypomethylation of miRNA encoding genes characterised DICER1/DGCR8-benign lesions, but as the tumors progressed to malignancy, methylation was partly reimposed, reversing the attempts to activate miRNA-encoded genes and further compromising miRNA production. Transcriptomic analyses revealed mutation-specific effects on the microenvironment, whereby DICER1 mutations activated canonical thyroid cancer progression pathways, whereas altered DGCR8 associated with immune-related alterations. This work unveils specific molecular events underlying malignant progression of miRNA-biogenesis related thyroid tumors and identifies potential biomarkers and disease etiology mechanisms. Together, these findings provide new insights into the molecular pathogenesis of familial and pediatric thyroid tumors, identify candidate susceptibility genes, and elucidate mechanisms of malignant transformation. This work contributes to advancing early diagnosis, risk stratification, and personalized management of thyroid diseases.
[spa] Los nódulos tiroideos son un hallazgo clínico común, y la mayoría son benignos. Sin embargo, ciertos nódulos pueden progresar hacia la malignidad, lo que plantea importantes desafíos clínicos en términos de diagnóstico, manejo y tratamiento de los pacientes. Las enfermedades tiroideas familiares y de inicio temprano, como la enfermedad nodular folicular tiroidea eutiroidea familiar (TFND, por sus siglas en inglés), son especialmente preocupantes, ya que pueden indicar una predisposición hereditaria al cáncer de tiroides. Aunque se conocen varios síndromes hereditarios bien caracterizados asociados al desarrollo de nódulos tiroideos, muchos genes de susceptibilidad siguen sin conocerse y el panorama genético completo es aún incierto. El microprocesador DICER1 ha sido implicado tanto en el desarrollo de TFND familiar como en tumores tiroideos pediátricos, así como recientemente también lo ha sido otro gen responsable del procesamiento de miARNs, DGCR8. No obstante, los mecanismos moleculares que subyacen a la progresión de lesiones benignas a malignas aún no están claros. Esta tesis doctoral pretende dilucidar las bases genéticas y moleculares de estas enfermedades a través de tres objetivos: (i) identificar nuevos genes candidatos que confieren susceptibilidad para la TFND eutiroidea familiar; (ii) caracterizar el perfil molecular de nódulos tiroideos pediátricos benignos y malignos en una serie española con el fin de identificar genes de susceptibilidad implicados en el desarrollo temprano de tumores tiroideos; y (iii) investigar los mecanismos responsables de la progresión de un nódulo tiroideo benigno hacia un cáncer de tiroides en lesiones con mutaciones en DICER1 y DGCR8. En conjunto, los hallazgos reportados en este trabajo de tesis aportan nuevos conocimientos sobre la patogénesis molecular de los tumores tiroideos familiares y pediátricos, identifican genes candidatos de susceptibilidad y dilucidan los mecanismos de transformación maligna en aquellas lesiones causadas por un defecto en la maquinaria de procesamiento de miARNs. Este trabajo contribuye al avance del diagnóstico precoz, la estratificación del riesgo y el manejo personalizado de las enfermedades tiroideas.
[cat] Els nòduls tiroïdals són una troballa clínica freqüent, i la majoria són benignes. No obstant això, alguns poden evolucionar cap a la malignitat, presentant reptes importants per al diagnòstic, la gestió i el tractament d’aquests. Les malalties tiroïdals familiars i d’inici precoç, com la malaltia nodular fol·licular tiroïdal eutiroïdal familiar (TFND), poden indicar una predisposició hereditària al càncer de tiroide. Tot i que s’han identificat alguns síndromes hereditaris associats, el marc genètic complet encara és desconegut. En aquest context, DICER1, un enzim clau per a la biogènesi dels microARNs (miARNs), ha estat implicada en la TFND familiar i en tumors tiroïdals pediàtrics. Les mutacions en DICER1 i altres gens com DGCR8 alteren el processament de miARNs, desregulen l’expressió gènica i afavoreixen la tumorigènesi. Aquesta tesi doctoral té com a objectiu investigar les bases genètiques i moleculars d’aquestes malalties a través de tres eixos: (i) identificar nous gens candidats de susceptibilitat en la TFND familiar; (ii) caracteritzar el perfil molecular de nòduls tiroïdals pediàtrics benignes i malignes; i (iii) estudiar els mecanismes que impulsen la progressió maligna en lesions amb mutacions a DICER1 i DGCR8. En primer lloc, es va fer la seqüenciació de l’exoma complet (WES) a 9 famílies amb TFND, identificant mutacions patogèniques en DICER1 i SLC26A4, i proposant MTHFR, WFS1, SELENOS i RB1 com a nous candidats, evidenciant l’heterogeneïtat de la malaltia. En segon lloc, el WES de 33 lesions pediàtriques va identificar impulsors genètics en lesions benignes (DICER1, PTEN, TPO, IYD, DUOX2, TSHR) i malignes (BRAF, fusions RET, DICER1), destacant el paper central de DICER1 en tumors pediàtrics. Finalment, l’anàlisi multiòmica va mostrar una acumulació progressiva de mutacions i una disminució de miARNs. Les lesions benignes presentaven hipometilació compensatòria, que es revertia parcialment amb la malignització. DICER1 activava vies tumorals conegudes, mentre que DGCR8 s’associava a canvis immunològics. En conjunt, aquest treball aporta noves dades sobre la patogènesi dels tumors tiroïdals familiars i pediàtrics, identifica nous gens de risc i ajuda a entendre els mecanismes de transformació maligna, amb implicacions importants per al diagnòstic precoç i la medicina personalitzada.
