dc.description.abstract[eng] This study presents a radiomics workflow based on T1-weighted MRI to differentiate lipomas from atypical lipomatous tumors in a retrospective cohort of 57 patients from Hospital de la Santa Creu i Sant Pau. The pipeline combined manual 3D segmentation, standardized preprocessing, extraction of 107 features using PyRadiomics, and statistical variable selection. Machine learning models trained with these features, specifically Support Vector Machines and Random Forests, achieved competitive performance consistent with the literature. The results relied on shape, volume, and texture descriptors, while basic clinical variables provided limited additional information. Despite these indications of technical feasibility and potential clinical value, the small sample size, lack of external validation, heterogeneity of imaging protocols, and absence of reproducibility and harmonization studies place the work at an intermediate level of methodological maturity (RQS2 ≈ 35%). Therefore, multicenter and prospective expansion is required prior to any potential translation into clinical practice. [cat] Aquest estudi presenta un flux de treball de radiòmica basat en RM ponderada en T1 per diferenciar lipomes de tumors lipomatosos atípics en una cohort retrospectiva de 57 pacients de l’Hospital de la Santa Creu i Sant Pau. El pipeline combina segmentació manual 3D, preprocesament estandarditzat, extracció de 107 característiques amb PyRadiomics i selecció estadística de variables. Els models d’aprenentatge automàtic entrenats amb aquestes característiques, específicament Support Vector Machines i Random Forests, van assolir un rendiment competitiu consistent amb la literatura. Els resultats es basaven en descriptors de forma, volum i textura, mentre que les variables clíniques bàsiques aportaven poca informació addicional. Tot i aquestes indicacions de factibilitat tècnica i potencial valor clínic, la mida reduïda de la mostra, l’absència de validació externa, l’heterogeneïtat dels protocols d’imatge i la manca d’estudis de reproductibilitat i harmonització situen el treball en un nivell intermedi de maduresa metodològica (RQS2 ≈ 35%). Per tant, és necessària una expansió multicèntrica i prospectiva abans de qualsevol possible traducció a la pràctica clínica.
