dc.description.abstractImmunoprofilaxi bacteriana en la prevenció de les infeccions urinàries recurrents en adults: revisió crítica de l’evidència clínica. Les infeccions del tracte urinari recurrents (ITUr) constitueixen una de les infeccions bacterianes més freqüents en la pràctica clínica i representen un problema rellevant de salut pública pel seu impacte en la qualitat de vida i per l’elevat consum d’antibiòtics. L’exposició repetida a antibiòtics afavoreix la selecció de bacteris resistents i ha impulsat l’interès per estratègies preventives alternatives. En aquest context, la immunoprofilaxi bacteriana ha emergit com una estratègia destinada a reduir la recurrència d’infeccions mitjançant l’estimulació de la resposta immune de l’hoste. L’objectiu d’aquest treball és analitzar críticament l’eficàcia clínica i el perfil de seguretat de les principals vacunes bacterianes utilitzades en la prevenció de les ITUr en adults. Es va realitzar una revisió narrativa crítica amb metodologia estructurada segons el model PICO. La cerca bibliogràfica es va dur a terme a PubMed/MEDLINE i Scopus entre octubre de 2025 i gener de 2026. Després del procés de selecció, es van incloure catorze estudis originals que avaluaven tres estratègies d’immunoprofilaxi: la vacuna sublingual polibacteriana MV140, la vacuna oral OM-89 i les autovacunes bacterianes individualitzades. Els resultats indiquen que totes tres intervencions s’associen a una reducció del nombre de recurrències d’ITU i del consum d’antibiòtics. Tanmateix, existeixen diferències en la solidesa de l’evidència disponible: OM-89 disposa d’assaigs clínics aleatoritzats amb efecte moderat, però consistent, mentre que MV140 presenta una evidència creixent amb reduccions potencialment més elevades però basada principalment en estudis observacionals. Les autovacunes mostren resultats prometedors en cohorts seleccionades, especialment en infeccions per microorganismes multiresistents (MDR). En conjunt, la immunoprofilaxi bacteriana podria representar una estratègia preventiva útil en el maneig de les ITUr i contribuir a reduir la dependència d’antibiòtics, tot i que són necessaris més estudis comparatius i assaigs clínics independents per definir-ne amb precisió el paper en la pràctica clínica. Bacterial immunoprophylaxis in the prevention of recurrent urinary tract infections in adults: a critical review of clinical evidence Recurrent urinary tract infections (rUTIs) are among the most common bacterial infections in clinical practice and represent an important public health problem due to their impact on quality of life and the high level of antibiotic consumption associated with their management. Repeated antibiotic exposure contributes to the emergence of antimicrobial resistance and has increased interest in alternative preventive strategies. In this context, bacterial immunoprophylaxis has emerged as a promising approach aimed at reducing infection recurrence through stimulation of the host immune system. The aim of this study was to critically analyze the clinical efficacy and safety profile of the main bacterial vaccines used for the prevention of rUTIs in adults. A structured narrative review was conducted following the PICO framework. A literature search was performed in PubMed/MEDLINE and Scopus between October 2025 and January 2026. After the screening process, 14 original studies evaluating three immunoprophylactic strategies were included: the sublingual polybacterial vaccine MV140, the oral bacterial lysate OM-89, and individualized bacterial autovaccines. The results suggest that all three interventions are associated with a reduction in UTI recurrence and antibiotic consumption. However, relevant differences exist in the strength of the available evidence. OM-89 is supported by randomized controlled trials showing moderate but consistent efficacy, whereas evidence for MV140 is increasing, with potentially larger reductions in recurrence rates but mainly derived from observational studies. Autovaccines have shown promising outcomes in selected populations, particularly in patients with multidrug-resistant infections. Overall, bacterial immunoprophylaxis may represent a valuable preventive strategy for the management of rUTIs and could contribute to reducing antibiotic dependence. Nevertheless, further independent randomized trials and direct comparative studies are required to better define the clinical role of these interventions.
