Robustness of resting-state functional MRI connectivity methods for Alzheimer’s disease biomarker identification

[eng] Alzheimer's disease (AD) is a progressive and irreversible neurodegenerative condition with no definitive cure. The identification of reliable, non-invasive biomarkers capable of detecting early disease stages and monitoring treatment response is therefore a critical need, particularly with the emergence of the first disease-modifying therapies. Resting-state functional magnetic resonance imaging (rs-fMRI) measures spontaneous functional connectivity fluctuations across brain regions, allowing the identification of large-scale functional networks whose alterations have been consistently associated with AD pathology, positioning derived measures as promising biomarker candidates. However, findings in the field are frequently inconsistent across studies, raising concerns about reproducibility and replicability that this project addresses.
A full rs-fMRI pipeline was developed and systematically evaluated across different subject selections, datasets, and network definition strategies using data from the Alzheimer’s Disease Neuroimaging Initiative, including cognitively normal subjects and patients with mild cognitive impairment. The pipeline includes preprocessing, quality control, group independent component analysis, network classification, dual regression, connectivity matrix computation, and statistical group comparison. The results demonstrate consistent functional connectivity differences between diagnostic groups across the tested configurations, particularly involving the default mode and executive networks. These findings were reproduced across different subject selections within the same dataset and were further supported by an atlas parallel analysis. Differences in sensitivity were observed across datasets, with larger datasets showing richer results, highlighting the importance of sample size and data quality on the reliability of rs-fMRI measures. Overall, the results provide encouraging evidence supporting the robustness and reproducibility of the proposed pipeline for AD biomarker identification.
[cat] L'Alzheimer (AD) és una malaltia neurodegenerativa progressiva i irreversible sense cura definitiva. La identificació de biomarcadors fiables i no invasius capaços de detectar estadis primerencs i monitoritzar la resposta al tractament és una necessitat crítica, especialment amb l'aparició de les primeres teràpies modificadores de la malaltia. La ressonància magnètica funcional en estat de repòs (rs-fMRI) mesura les fluctuacions de connectivitat funcional espontània entre regions cerebrals, identificant xarxes funcionals a gran escala, les alteracions de les quals s'associen a la patologia de l'AD. No obstant això, els resultats en el camp són freqüentment inconsistents entre estudis, generant preocupacions sobre reproductibilitat i replicabilitat que aquest projecte aborda directament. Es va desenvolupar un pipeline complet de rs-fMRI avaluat sistemàticament en diferents seleccions de subjectes, conjunts de dades i estratègies de definició de xarxes, utilitzant dades de l'Alzheimer's Disease Neuroimaging Initiative, incloent subjectes cognitivament normals i pacients amb deteriorament cognitiu lleu. El pipeline inclou preprocessament, control de qualitat, anàlisi de components independents grupal, classificació de xarxes, regressió dual, càlcul de matrius de connectivitat i comparació estadística de grups. Els resultats demostren diferències consistents de connectivitat funcional entre grups diagnòstics, especialment a les xarxes de mode per defecte i executiva. Els resultats es van reproduir en diferents seleccions de subjectes i van ser recolzats per una anàlisi paral·lela basada en un atles. Es van observar diferències de sensibilitat entre conjunts de dades, destacant la importància de la mida de la mostra i la qualitat de les dades. En conjunt, els resultats aporten evidència encoratjadora sobre la robustesa i reproductibilitat del pipeline proposat per a la identificació de biomarcadors de l'AD.
