SIBO: de la recerca a la consulta

[cat] Introducció: El sobrecreixement bacterià a l’intestí prim és un trastorn intestinal caracteritzat per una proliferació anormal de bacteris a l’intestí prim que comparteix simptomatologia amb un seguit de trastorns gastrointestinals. Objectius: Analitzar l’eficàcia i conèixer les limitacions dels mètodes actuals de diagnòstic i tractament del SIBO, i identificar tendències i patrons dietètics en quant als mètodes de tractament usats. Conèixer l’experiència del pacients amb SIBO al llarg del procés de diagnòstic i tractament, així com, el tracte rebut per part dels professionals de la salut. Finalment descobrir l’impacte que té en la qualitat de vida dels afectats. Materials i mètodes: Inclou la cerca bibliogràfica en les bases de dades de Pubmed, Scopus i Google Scholar, i la participació de 308 individus afectats pel SIBO en la realització d’una enquesta, a través de la difusió del formulari en diferents associacions i centres especialitzats d’Espanya. Resultats: Per una banda, existeix una gran variabilitat en les metodologies usades en les proves diagnòstiques i en la interpretació dels resultats, així com en les dosis i durades administrades de les opcions terapèutiques estudiades fins al moment. D’altra banda, es qualifica molt negativament l’atenció rebuda pel sistema de salut, remarcant l’afectació que té aquest trastorn en la qualitat de vida dels que el pateixen. Conclusions: Són necessaris més d’estudis amb major mida mostral, amb simptomatologia definida i criteris consensuats per diagnosticar i guiar el tractament eficaç del SIBO. L’abordatge integral i accessible a tothom ha de ser essencial per garantir una atenció de qualitat i equitativa a tots els pacients.

[eng] Background: Small intestinal bacterial overgrowth is an intestinal disorder characterized by an abnormal proliferation of bacteria in the small intestine that shares symptomatology with a number of disorders. Objective: To analyze the efficacy and to know the limitations of current methods of diagnosis and treatment of SIBO, identifying trends and dietary patterns in terms of treatment methods used. To know the experience of patients with SIBO throughout the diagnosis and treatment process, as well as the treatment received by health professionals. Finally, to discover the impact on the quality of life of those affected. Methods: In addition to the bibliographic search in Pubmed, Scopus and Google Scholar databases, 308 individuals affected by SIBO participated in a survey, through the dissemination of the form in different associations and specialized centers in Spain. Results: On the one hand, there is great variability in the methodologies used in the elaboration and interpretation of the results of the diagnostic tests as well as in the doses and durations of the therapeutic options administered so far. On the other hand, the care received by the health system is rated very negatively, highlighting the affectation that this disorder has on the quality of life of those who suffer from it. Conclusions: More studies with larger sample size, clearly defined symptomatology and consensus criteria are needed to diagnose and guide effective treatment of SIBO. A comprehensive and universally accessible approach is essential to ensure quality and equitable care for all patients.
