Resum

[eng] Introduction: People with kidney failure have a higher risk of cancer compared to age- and sex-matched individuals in the general population, especially after kidney transplantation. Moreover, mortality after a cancer diagnosis is also higher than expected in the general population. Few studies have focused on the epidemiology of cancer in the kidney failure population in Southern Europe. The risks described in other regions might not be entirely generalizable to our population, due to differences in the management of chronic kidney disease, access to medical care, or environmental exposures. Current evidence describing cancer-related mortality in people with kidney failure provides an incomplete understanding of the drivers of mortality in this population. Competing risk mortality estimates in people with kidney failure use deaths where cancer is the primary cause, and do not account for cases when cancer may be an underlying cause of death, potentially underestimating cancer-related mortality. Relative survival can provide insight into the excess mortality, directly or indirectly, attributed to cancer in the kidney failure population. Hypothesis: Understanding the risks and outcomes in this population, along with the factors contributing to excess cancer risk and mortality, will provide valuable insights into the epidemiology of cancer in the population with kidney failure and help design strategies to improve its prevention and detection in this population. Objectives: • To describe the incidence of cancer in people on dialysis and kidney transplant recipients compared to the general population in Catalonia. • To identify individuals with the highest excess risk of cancer among the Catalan kidney failure population. • To estimate and compare all-site and site-specific relative survival in the kidney failure population with cancer. • To evaluate relative survival trends over time and by patient factors in the kidney failure population with cancer. • To identify factors contributing to excess mortality in people with kidney failure and cancer compared to the general population with cancer.
